* refactor(config): default icons to ascii
Problem: default icons used unicode characters (○, ✓, ●, ▸, ·, ↺)
which render poorly in some terminals and font configurations.
Solution: replace defaults with ascii equivalents (-, x, !, >, ., ~).
Users can still override to unicode or nerd font icons via config.
