docs: add upstream tracker
Problem: the full upstream PR and issue triage was removed from the README during the repository modernization, leaving no user-facing record of what the fork addresses. Solution: restore the triage tables in a dedicated doc/upstream.md and link to it from the top of the README. Keeps the README clean while giving the full picture a permanent home.
